Set Up Structured Evaluation Forms for Candidate Reviews
Overview
Structured Evaluation Forms help your team provide consistent, high-quality feedback when reviewing candidates.
Instead of relying on free-form comments alone, you can create standardized questions and rating criteria that reviewers complete during the review process. Evaluation forms can be configured for:
- Review stages within an opening's hiring workflow.
- Quick reviews sent at any point in the hiring process.
Using structured evaluations helps ensure reviewers assess candidates against the same criteria, making feedback easier to compare and supporting more consistent hiring decisions.
Set Up an Evaluation Form for a Review Stage
You can attach a Structured Evaluation Form to any Review stage in an opening's hiring workflow.
To configure an evaluation form:
- Go to Openings.
- Select the opening you want to configure.
- Open the opening details.
- Click Manage Hiring Workflow.
- Either:
- Add a new Review Stage, or

Select the Edit (pencil) icon next to an existing review stage.

Click the Set up Evaluation Form button for the stage

Add Evaluation Criteria
Create the questions or competencies reviewers should evaluate.
To add an evaluation item:

Click Add an Attribute / Quality.

Enter the evaluation criterion.

Repeat until all desired questions have been added.
Examples include:
- Communication skills
- Technical knowledge
- Problem solving
- Leadership
- Culture add
- Role-specific competencies
You can edit or delete evaluation items at any time before saving the form.
Copy an Existing Evaluation Form
If you've already created an evaluation form for another opening or review stage, you don't have to recreate it manually.
Click Copy Evaluation Form to reuse an existing template, then make any necessary adjustments for the current review stage.

Then you can select the opening and stage where the evaluation form that you want to copy is.
Make Evaluation Items Required
You can require reviewers to complete specific evaluation criteria.
To make an attribute mandatory:

Select the Mandatory checkbox for the attribute.

When an attribute is marked as mandatory:
- A rating is required.
- Comments are also required before the review can be submitted.
Preview and Save the Evaluation Form
Before publishing your changes:

Preview the form to verify the layout and questions.

Click Done then Save on the review stage.

Once saved, every candidate who enters that review stage will be reviewed using the configured evaluation form.
Use an Evaluation Form for a Quick Review
Structured Evaluation Forms can also be used when sending a review outside of the hiring workflow.
To create a quick review:
- Open the candidate's profile.

Click Review then Edit evaluation form.

Configure the evaluation form for the review request.

- Send the review to the selected user.
The reviewer will complete the structured evaluation along with their overall review feedback.
Important: Keep in mind that evaluation forms created through the quick review feature won't have the option to copy from another form or be saved in the same way as evaluation forms created in a Review stage.
Review Submitted Feedback
When reviewers complete the evaluation:
- Ratings are recorded for each evaluation criterion.
- Comments are captured for each question.
- Feedback is stored on the candidate's profile.
To review submitted evaluations:
- Open the candidate's profile.

Select the Feedback tab.

All submitted review feedback can be viewed there for future reference and hiring discussions.
Best Practices
To get the most value from Structured Evaluation Forms:
- Use the same evaluation criteria for similar roles.
- Focus questions on job-related skills and competencies.
- Require ratings and comments for critical evaluation areas.
- Reuse evaluation forms across openings when appropriate to promote consistency.
- Periodically review and update evaluation criteria as hiring needs evolve.
